Join Our Better Breathers Club
We invite you to join our Better Breathers Club, a supportive community for individuals living with lung disease. All meetings are held via Zoom on the 2nd Wednesday of every month at 11:30 AM PST. It’s a great opportunity to connect with others, share experiences, and gain valuable insights into managing lung health.

Physician Determines Hospice Care Eligibility
Physician Orders Hospice Services
a Dynamic Transitional Care Coordinator will meet with the patient and family or connect over the phone to coordinate care.
Our Nursing team conducts a comprehensive assessment of the patient to initiate Hospice Care and begin scheduling team visits.
The Dynamic Hospice Care team schedules visits based on the patient's Plan of Care (POC) and individual needs.
